This clinic has been established to specialise in the treatment and management of both cosmetic and complex skin conditions including skin aging, skin disease and skin damage, as well as to improve peoples health as a whole.

In addition to professional skincare, aesthetic procedures and phototherapy, Medacare specialises in functional medicine and nutritional dermatology which involves custom formulated topicals, dietary interventions and prescription supplementation. This is to reduce the use of pharmaceuticals for skin problems, as well as improve treatment safety and longterm success.

Functional medicine is different to conventional medicine, it recognises disease and aging is a continuum, not a simple point in time in a persons history of health. It also recognises an individual’s biological uniqueness and takes into account their personal health history, disease progression and treatment preferences. It focuses on identifying and treating the aetiology of a disease or disorder (the cause, set of causes, or manner of causation), rather than just treating the pathogenesis with a drug, treating only the processes that maintain it. This could be age, systemic health conditions, maladapted stress responses, injury, allergy or chemical exposure, infection, deficiency or impairments of normal biological skin mechanisms.

By identifying and addressing all cause, triggers and symptoms synergistically with patient focused care and more suitable physiological compounds, we can treat both the appearance and underlying cause of your condition, and you as a whole, to achieve the greatest clinical results.

The follow-up review is an important part of treatment and a chance to discuss the outcome of the initial prescription, procedures and dietary adjustments, as well as make any necessary changes to ensure you continue to receive the most relevant treatment as the condition improves or if symptoms change.

With chronic skin conditions, we need follow-ups to monitor the frequency, duration and intensity of flares, this helps to recognise progress, stay anchored in the process and appreciate the trajectory.

Getting out of the flare isn't the only problem, we need to have identified during this process, factors in the patients biology that can help them move away from the condition, build biological robustness so it dosn't keep recurring.

How many follow-up visits are needed depends on how long the condition has been present, the severity of the condition and how well the prescription is adhered to.

IPL MODERATE LEVEL LIGHT THERAPY

Treatment on the Anthélia can be performed using moderate level pulsed light (less energy and heat), also known as moderate level light therapy (MLLT). When MLLT is performed, oxyhemoglobin (blood) is the main target where hemoglobin is heated to levels between 50ºC and 60ºC with filtered light (between 475nm to 1100nm) to rebuild the skin.

Moderate level pulse light is a gentle non-ablative treatment which does not violate the epidermis or cause social downtime, instead it stimulates platelet activation, growth factors and cell proliferation to improve cutaneous micro-relief and augment skin remodelling through the repair of the extracellular matrix (ECM).

The ECM constitutes over 70% of the skin and is the central hub for repair and regeneration of the skin. As such, the ECM is the area where changes related to photodamage are most evident. Degradation of the ECM and accumulation of collagen fragments, amorphous elastin agglutinations, and abnormal cross-linkages between the collagen fragments impedes the ECM from its normal repair and regenerative capacity, which manifests as wrinkled, non-elastic skin.

The outcome is facial lifting and body smoothing, also known as phototoning (Phototensor®). Repairing the ECM reduces rhytids (lines and wrinkles), increases hydration capability and capacity, improves skin laxity, elasticity, density and fragility. This treatment stimulates the natural production of collagen, elastin and glycosaminoglycans (GAGs) which are the body's natural moisturisers, tighten the skin and dilated pores for a smooth skin texture, and improve vascular health and tone, all contributing to the rejuvenation of aging or damaged skin.

IPL HIGH LEVEL LIGHT THERAPY

Treatment on the Anthélia can also be performed using high level pulsed light (more heat), also known as high level light therapy (HLLT) to remove or reduce unwanted cosmetic abnormalities such as pigmentation, red veins and viscosities, forms of dermatosis as well the removal of unwanted hair through selective photo-coagulation and photo-thermolysis.

When treatment is performed using HLLT (more heat and energy), melanin (pigment) and oxyhemoglobin (blood) are the main targets where they are heated to levels above 80ºC to 90ºC with filtered light (between 475nm to 1100nm) to eliminate imperfections in the skin.

This causes selective necrosis of pigment, photocoagulation of blood and contraction of collagen fibres on abnormal blood vessels, for the destruction and removal of unwanted cosmetic abnormalities and manifestations such as pigment, red veins, forms of dermatosis and even infection progressively disappears.

Where acne blemishes are involved, oxyhemaglobin, sebaceous glands and porphyrins are the main target, where hemoglobin is heated to above 80ºC using filtered light (between 475nm to 1100nm) causing selective photocoagulation, a reduction in vascularisation of the sebaceous glands, reduced sebum production, inflammatory acne and infection through an antibacterial action.

Just like repairing the skin, removing all the damage is a process that progresses in stages. The pulsed light, in complete safety, eliminates excess melanin present in the epidermis, facilitating the disappearance of pigmentation and targets the hemoglobin in red blood which transmits to the vessel walls causing coagulation and elimination by the body. The photo-thermal effect of pulsed light will accelerate the natural renewal of the cells of the epidermis to give it a uniform complexion and a homogeneous colour. Thanks to this treatment, the skin can regain a clearer more youthful appearance in the treatment area.

SESSIONS AND INTERVALS

Visible results can be seen from the very first session, amplifying with each successive session until the desired outcome is achieved and retained with appropriate maintenance sessions.

As a one-off photofacial you can improve your skins texture and complexion (radiance/pigmentation/vascularity) but a single one-off treatment is not enough to improve rhytids (lines and wrinkles) and conditions associated with the breakdown of the ECM. For best results 5 to 6 sessions is the average number of sessions required (remove, restore, rebuild).

A minimum recovery and rest period of 2-3 weeks between sessions is the most common interval to apply when treating multiple skin presentations during a course of successive treatment.

The shortest interval you can afford (time/budget) is best for results but good results can still be achieved when intervals need to be a little more spread out (example 4-6 weeks) to economize treatment.

RECOVERY AND RENEWAL

No social downtime unless pigmentation and viscosities removal are included. Renewal sessions can be performed as needed or at regular intervals of 3 to 6 months to maintain optimal results and prevent new skin damage from accumulating. TOPICAL LOCAL ANAESTHETIC (TLA)

Topical local anaesthetic (TLA) leads to a superficial loss of sensation over the skin where it is directly applied. Unlike local anaesthetics, TLA does not require painful injections and is topically administered on the skin via a cream. TLA works by reversibly blocking the nerve conduction of the nerve cells within the dermis of the skin. TLA disrupts the nerve cell permeability to sodium, decreasing polarisation and subsequently impeding the nerve cells ability to generate an action potential.

ADVERSE EFFECTS TO CONSIDER

TLA in rare cases can cause a burning or stinging sensation at the site of administration, if this occurs the cream can be removed.

Rarely TLA may also lead to an allergic reaction, if you are allergic, this procedure cannot be performed at Medacare, patch testing before the day of your procedure can test for this risk.

If TLA is used in excess, it can cause systemic toxicity, this is why there is a strict limit on the amount of TLA that can be used during one procedure to mitigate this risk. Medacare will not budge on this which may limit how much work can be done with Plasma in a single day if you have multiple or large areas to treat, multiple appointments will be needed.

IS TLA ESSENTIAL

Yes, to reduce the pain, discomfort and sensation of the treatment so that you can comfortably lie still for controlled and accurate plasma to be performed, especially in delicate sensitive areas.

FOLLOW-UP AND SUPERVISION (POST CARE)

It takes at least 14 weeks to see final results and complete the healing cycle. Changes are likely to be subtle from day to day and more dramatic as they cumulate over time. Improvements may continue for up to 6-12 months. The healing cycle unfolds in two stages over 3 months.

THE HEALING PROCESS

Plasmage Healing Stage 1: DOTS

Stage one of Plasmage healing is when scab dots are on the treatment area. Face dots shed approximately 4 - 14 days following treatment (average client begins to shed dots on day 7). Dots on the body often take longer than the face, potentially around 2-3 weeks.

No sun! Avoid all sun exposure by keeping a physical barrier between your treatment area and sunlight during Stage 1 of healing.

Pat dry - keep dry! No Products, Liquid, or Steam! Keep area clean and dry. If needed, clean treated area using a mild, unscented, alcohol-free cleanser such as Cliniprep. Do not apply sunscreen or makeup (even mineral makeup) until the scabbing has fallen off completely. This is to avoid infection or disrupting the scabs prematurely. Avoid exercising immediately after a treatment, as any heat, steam, or sweat can add to inflammation and disturb dots. Do not use any pools, hot tubs, or saunas until after the dots have fallen.

Keep the dots on! No Picking or Friction on Scabs! Prematurely removing scabs may result in scarring and discoloration. Allow scabs to come off on their own. This will ensure the best possible result and avoid scarring, hyperpigmentation, and infection. The longer the scabs stay put, the longer the skin has a chance to heal itself with the protection of the scab. Use a 100% silk pillowcase and silk eye mask for sleep to avoid scab friction.

You may resume most activities immediately after any treatment, but you may not look your best during healing stage one while the dots are on the treatment area. Depending on treatment location and comfort level, you may choose to avoid in-person appointments during this time.

If needed, use ice or anti-inflammatory herbs, and/or oral antihistamines to help with swelling and/or itching.

Avoid shaving over any treated area until the skin has fully healed.

Avoid smoking.

Plasma Healing Stage Two: DOTS NO MORE!

Stage two begins when the last dot has fallen and continues until 3 months from the date of treatment has passed. You may apply makeup at this stage and slowly return to your normal skin routines.

Always apply broad-spectrum SPF 50+ to the treatment area until at least 3 months after your date of treatment. Healing skin can become damaged by UV rays, causing hyperpigmentation and/or premature aging.

Update SPF 50+ every 80 minutes.

Avoid harsh products (like acids, peels, or physical exfoliants) until the 8 weeks post-treatment healing point. Use common sense: if it doesn’t feel good, don’t do it. Patch test and re-introduce active ingredients slowly.

HEALING PROCESS DAY BY DAY

Day 1: Try to lay propped up with pillows. Some discomfort is normal. Swelling (if present) will likely be worse in the mornings and lessen towards the evening. It will feel like a bad sunburn.

Day 2: Swelling typically peaks today. Still feeling like a sunburn. This too shall pass.

Day 3: You may notice an improvement in the swelling and the “dots” will start to crust/scab over. Allow scabs to fall off on their own. Discomfort will have subsided a bit.

Day 4: Discomfort should continue to diminish.

Day 5: Scabs may still be present.

Days 6 - 14: Wait patiently for the remaining scabs to fall off on their own. It is normal for the new skin under the scabs to be pink or red.

Weeks 2 - 8: After scabs fall off, you may have tender, peeling or textured skin for a few days or weeks. Once tenderness subsides, use a non-physical, gentle, enzyme exfoliant to slough off peeling skin. After scabs fall off, you may also use healing serums, lotions and make up. Each day, your skin will progressively return to its’ original coloration and texture.

Depending on laxity and desired results, you may have multiple treatments on the same area. Areas may be treated every 3 months to let the collagen heal and turn over.
